Participation Consent Form
 
I understand that this group provides support, not therapy or counseling. 
 
Furthermore, as a participant in this group, I will respect the confidentiality of all information shared by other members.  I also expect that what I discuss will be held in utmost confidence by the facilitator(s) and other participants, except when the following legal limitations apply:
 
1. The participant expresses intent to harm themselves or another person.
 
2. There is reasonable suspicion that abuse or neglect toward a child, elder, or dependent adult has occurred.
 
3. When ordered by a court.
 
Finally, I agree to hold the Pet Loss Community and Pet Loss Community's facilitators harmless from and against any and all liabilities.
